STEP 1 - COMPLETE THE APPLICATION FOR ADMISSION FORM
Complete and sign the Application for Admission
Form and submit it along with the following requirements. You may also
fill-up the Application for Admission Form through our on-line system. * Our on-line system provides convenient way in
filling-up the Application for Admission Form. Applicants using this
system must still present the required documents to be fully accepted by
Philippine Pilot Training Center. VISA: http://www.dfa.gov.ph/
STEP 2 - LET YOUR PHYSICIAN OR DOCTOR CHECK YOUR PHYSICAL FITNESS AND
FILL-UP THE PHYSICAL RECORD FORM. The
Physical Record Form must be completed and mailed by your physician or
doctor directly to the school. Applicants are not allowed to have it in
their personal possession. STEP 3 - OBTAIN AN AIRMAN
MEDICAL CERTIFICATE You may contact the Civil Aviation Authority in
your country to request for a designated flight surgeon within your
area. An Airman Medical Certificate will be issued by a flight surgeon
